Release form for bodies being transferred to another funeral home. Upon completion of this form, it can be faxed to 409-986-9903, or scanned and sent via email by clicking here.

The State of Texas will contact the family directly for all death certificate information. Carolyn McMillan is the contact for providing death certificate information. She can be contacted by clicking here.

Carnes Funeral Home will need to be contacted regarding ordering Death Certificates for all deceased inmates. To Order Death Certificates, a written request will need to be submitted, with payment attached.
